1、桂來高速公路5標項目部技術交底 第 9 頁 共 9 頁 記錄表01 施工技術交底記錄表承包單位：湖南路橋建設集團公司 合同段：5監(jiān)理單位：廣西八桂工程監(jiān)理咨詢有限公司 編號：工程名稱路基邊坡開挖施工樁號及部位K1348+300K1348+600交底內容： K1348+300K1348+600路基邊坡開挖施工方案一、施工內容待開挖邊坡已全部被灌木叢覆蓋，安全穩(wěn)固，為進行樁基礎施工，已進行邊緣小部分土方開挖，見下圖。 邊坡正面圖 邊坡側面圖邊坡高8米左右，坡度約為1:1.5，原有漿砌片石骨架防護構造物，由于該地區(qū)降雨豐富，因此植被茂盛，邊坡已全部被灌木叢覆蓋，中間夾雜有35米高的喬木，截水溝已被灌

2、木叢覆蓋，截水狀況良好，現(xiàn)擬進行覆蓋灌、喬木砍伐及清表工作。二、公路運營安全防護措施1、設置砂袋墻（見下圖）根據(jù)我項目的施工條件，結合現(xiàn)場實際調查，為保證K1348+300K1348+600路基邊坡開挖施工過程中不影響原柳南高速公路的正常通車，避免在施工過程中對原柳南高速公路正常通車造成安全隱患，對于砍除原挖方邊坡的樹木采用在原高速公路防撞護欄外側堆積砂袋墻的安全防護形式，砂袋墻高1.5，寬0.5米?？柯访孢@一側采用高2米的藍色彩鋼瓦包邊。防護橫斷面圖彩鋼瓦擋板示意圖砂袋墻平面示意圖2、砍筏邊坡樹木在砍筏中應采取繩拉或控制刀口方向等措施使樹木不倒向公路路面方向，砍伐過程中嚴禁亂丟煙頭，防止發(fā)生

3、火災。3、設置防落網(wǎng)（見下圖）開挖邊坡時，在原有邊坡修筑2米平臺，每隔4米安裝一處鋼管三腳架，上鋪防落網(wǎng)，以防止開挖過程中土石塊滑落到原高速公路上。在路基坡頂挖方施工時，緊鄰老邊坡位置預留1米寬不開挖，待分層挖深12米時，再由外向內開挖原預留的1米，減少落土石的幾率，同時及時檢查開挖后砂袋墻，對松動的土石及時進行清理。防落網(wǎng)示意圖4、開挖前先進行灌木、喬木砍伐，再進行清表，先進行A、E匝道線路開挖，待樁基礎完工后再進行D、H匝道線路開挖，開挖前應設置臨時排水溝，防止沖刷的泥土污染高速公路路面。三、一般施工安全技術措施一般施工安全技術措施1、施工機械作業(yè)時，除按規(guī)范操作外并應按事先設計的行走路線

4、進行，其工作位置應平坦穩(wěn)固，并應有專人指揮，指揮人員不得進入機械作業(yè)范圍內。2、挖方邊坡實行“誰開挖、誰加固、誰保護”的原則，施工時嚴格按照設計方案進行施工。3、邊坡施工作業(yè)人員必須戴好安全帽，系好安全帶，綁掛安全帶的繩索牢固地拴在可靠的安全樁上，繩索應垂直，不得在同一個安全樁拴2 根及以上安全繩上拴2人以上。4、邊坡施工應設置安全通道；開挖工作面應裝運作業(yè)面相互錯開，嚴禁上、下交叉作業(yè)，邊坡上方有人工作時，邊坡下方不準有人停留或通行。5、清理邊坡上突出的土石和整修邊坡時，應從上而下順序進行，坡面上的松動土、石塊必須及時清除。嚴禁在危土石下方作業(yè)、休息和存放機具。6、施工中如發(fā)現(xiàn)山體有滑動、崩

5、塌跡象危及施工安全時，應立即停止施工，撤出人員和機具，并報告監(jiān)理辦和指揮部處理。7、滑坡地段的處理，應從滑坡體兩側向中部自上而下進行，嚴禁全面拉槽開挖。施工中要設專人觀察，嚴防塌方。8、遇有大雨、大霧及六級（含六級）以上大風等惡劣天氣時，應停止作業(yè)。邊坡路堤下方道路，施工時應設置警示標志。9、施工機械靠近路堤邊緣作業(yè)時，應根據(jù)路堤高度留有必要的安全距離（距離邊緣1米以上），并設專人指揮，指揮人員不得進入機械作業(yè)范圍內。10、棄土下方和有滾石危害范圍道路，應設警告標志、作業(yè)時提醒下方車輛通行。四、安全施工專項措施安全施工專項措施1、必須按設計規(guī)范施工。2、全部作業(yè)過程嚴格執(zhí)行操作規(guī)程。3、路基施

6、工準備（1）、機械設備配備：挖掘機2臺、裝載機1臺、推土機1臺、壓路機1臺、平地機1臺、自卸翻斗車16臺、供電設施一套、照明設備一套、汽車吊1臺。（2）、安全防護用品：安全帽、警示牌、口哨、指揮旗、標志牌、警示燈、安全帶等。4、開工前檢查：挖掘機、裝載機、推土機、壓路機、平地機、自卸翻斗車、供電設施、照明設備、汽車吊等是否處于良好狀態(tài)，各項制動是否有效，電纜線有無破損等情況。5、施工安全注意事項（1）、在靠近高速公路交通繁忙地段施工，需安排專人進行警戒。（2）、車輛通過較多、運輸較繁忙的便道，彎道半徑小于15m，特殊地段小于10m須掛安全隔離標志帶及警示牌。（3）、施工現(xiàn)場便道急彎處、陡坡地段

7、要求懸掛標志標識牌。（4）、嚴禁穿硬底、帶釘子、易滑、高根、拖鞋或赤腳進入施工現(xiàn)場。（5）、施工現(xiàn)場材料、設備擺放整齊有序。（6）、施工翻斗車不能行車載人及超載超速。其它施工機械不得超速行駛及違章作業(yè)。前后兩車（機械）間距不應小于10m。（7）、路塹開挖嚴禁采取掏底開挖（忌挖神仙洞），以免坍塌。（8）、嚴禁在松動危石下、未熄火的大型機械旁作業(yè)和休息。（9）、棄土場的選址需避免泥石流溝。（10）、嚴禁在山坡上同一地段的上下不同時作業(yè)。（11）、人工挖掘作業(yè)人員橫向間距不應小于2m，縱向間距不應小于3m。（12）、邊坡開挖須從兩側向中部自上而下開挖，禁止全面拉槽開挖。（13）、邊坡開挖施工前必須做

8、好邊坡截水溝，防止雨水對邊坡的沖刷，造成邊坡土石方滑溜，對作業(yè)人員造成危險和影響既有線的行車安全。（14）、邊坡開挖后要用彩條布等及時覆蓋，防止雨水沖蝕，坡體失穩(wěn)坍塌。并確保運營中柳南高速公路兩邊水溝的排水暢通。在反向開挖、砍除原邊坡防護樹木的同時，杜絕樹木滑落到原高速公路上，局部有危險段或者開挖不到得位置處，采用人工拆除，臨時搭建升降機，運送拆除后的樹木，對于砍除后的樹木應集中堆放后，就近運至桂來高速公路K233+400處進行處理。五、安全生產(chǎn)組織機構及領導小組1、組織機構安全組織機構（二）領導小組（含聯(lián)系方式）組 長：雷榮華（18674896246）副組長：成 曉（13786262808）

10、應對各種安全突發(fā)狀況；李征斌（13755169908）為現(xiàn)場施工負責人，負責現(xiàn)場的施工組織工作，并協(xié)助劉國強做好安全管理工作。六、突發(fā)事件應急處置措施根據(jù)挖方段現(xiàn)場實際情況，可能發(fā)生的突發(fā)事件有火災、交通事故等。1、火災應急處理措施 (1)任何人發(fā)現(xiàn)火災時，應立即報警，報警人員在報警時應同時說清著火地點、部位、燃燒物品、火災狀況等。同時報告所安全領導小組。通知相關部、室負責人，同時做好滅火前的必要準備工作，及時記錄火災情況。(2)接報告后，所有義務消防隊成員必須無條件及時趕赴現(xiàn)場，參加救火行動。(3)全體在場工作人員、義務消防隊成員必須立即加入撲救火災行動。在滅火救災指揮部指揮下，疏散救護工作

11、要有條不紊，責任到人，由各安全員負責進行引導護送，向安全區(qū)疏散，同時要查清現(xiàn)場是否有遺漏人員。接應救護車，指引救護人員及時對傷員進行處理或送醫(yī)院救治。(4)現(xiàn)場成立滅火救災指揮部，由工程部長及事發(fā)部門負責人組成，工程部長任總指揮。其主要職責是：負責查看火情，掌握火勢發(fā)展情況，隨時向119指揮中心通報火情，根據(jù)火情指揮切斷電源、可燃性氣體源，指揮義務消防隊和增援人員滅火，指揮搶救傷員，疏散物資，及時控制火勢蔓延。在向119報警后，及時派出人員接應消防隊和清除交通通道障礙，疏散圍觀群眾，做好警戒工作。公安消防隊到達后，及時向公安消防火場總指揮報告災情，按統(tǒng)一步驟，組織職工貫徹執(zhí)行，做好安全工作。(

12、5)現(xiàn)場指揮員有權根據(jù)撲救火災的需要，決定如下事項：使用各種水源，限制用火用電直至停止使用，劃定警戒區(qū)。(6)根據(jù)現(xiàn)場具體情況劃分安全警戒線，安全警戒線分為建筑物外圍警戒和火災現(xiàn)場警戒。 外圍警戒要及時消除路障，勸阻無關人員、車輛離開現(xiàn)場，維持好建筑物外圍秩序，為公安、消防隊到場展開滅火創(chuàng)造有利條件。火災現(xiàn)場警戒要及時指揮疏散人員，看管好搶救出來的物件。2、交通事故應急處理措施應立即撥打122報警電話;市公安交警部門接警后應立即趕赴現(xiàn)場,處理交通事故或車輛拋錨情況應在事故發(fā)生地前端100米處設置警示標志,并立即通知公安交警部門對高速公路實行交通管制。應記錄肇事車輛的車牌、車型、顏色及駕駛員的姓
