2、期準(zhǔn)備過程中的是極為重要的一個(gè)環(huán)節(jié)，存在著極大的危險(xiǎn)性，一旦控制不好，不但會(huì)影響盾構(gòu)的正常始發(fā)，還將對作業(yè)人員的人身安全造成威脅，并可能造成項(xiàng)目財(cái)產(chǎn)的巨大損失。為充分利用項(xiàng)目資源，合理規(guī)劃運(yùn)作方法，以處理在洞門鑿除施工作業(yè)過程中可能發(fā)生的各種緊急情況，實(shí)施在發(fā)生重大生產(chǎn)安全事故時(shí)的救援和事后處理工作，盡量減少事故的危害和影響，保障項(xiàng)目部施工人員、周邊居民和可能受事故影響人員的人身健康和安全，保障項(xiàng)目部、周邊單位和公眾財(cái)產(chǎn)的安全，特制定本預(yù)案。1.3適用范圍本應(yīng)急救援預(yù)案僅適用于本項(xiàng)目右線盾構(gòu)始發(fā)洞門鑿除過程中可能發(fā)生的各類險(xiǎn)情及安全事故的應(yīng)急救援及處置工作。各部門及相關(guān)人員將依據(jù)預(yù)案要求，對相

3、關(guān)措施組織學(xué)習(xí)。二、 工程簡介本工程范圍為深圳地鐵一號線（羅寶線）續(xù)建工程土建20標(biāo)段固戍站后瑞站地下段土建工程右線盾構(gòu)區(qū)間。固戍站后瑞站區(qū)間起止里程為SK35+037.20SK36+800.00，右線盾構(gòu)隧道總長1762.163m（短鏈0.637m）。區(qū)間線路位于深圳市寶安區(qū)寶安大道下方，線路南北走向，盾構(gòu)機(jī)從明挖段南端（里程SK36+800.00）下井始發(fā)，沿線穿行寶安大道下方，中間通過下錨段明挖施工段（里程范圍SK36+000.000SK36+061.600,總長61.6m），到達(dá)固戍站北端頭（里程35+037.20）解體吊出，完成掘進(jìn)施工。管片外徑6000mm，內(nèi)徑5400mm，管片厚

4、度300mm，管片寬度1500mm，混凝土強(qiáng)度C50，管片拼裝形式為錯(cuò)縫拼裝。三、 施工作業(yè)過程中的危險(xiǎn)性分析1、 作業(yè)人員作業(yè)過程中未佩戴好安全防護(hù)用品，造成傷害事故；2、 洞門鑿除作業(yè)人員未持證上崗，在接電及鋼筋切割過程中，違章操作引發(fā)事故；3、 洞門鑿除搭設(shè)作業(yè)平臺不穩(wěn)，作業(yè)時(shí)坍塌造成作業(yè)人員傷害事故；4、 作業(yè)前未進(jìn)行專項(xiàng)交底及安全技術(shù)交底，作業(yè)過程中不按照方案施工、野蠻施工，造成事故；5、 洞門鑿除過程中動(dòng)火作業(yè)及明火作業(yè)未辦理動(dòng)火審批，防護(hù)措施不到位，造成火災(zāi)事故；6、 洞門鑿除過程中交叉作業(yè)未作好防護(hù)措施，造成物體打擊事故；7、 做檢測孔檢測過程中，有泥沙或水流出，未及時(shí)采取措施

5、，造成坍塌事故；8、 在剔除洞門上不穩(wěn)定的水泥塊時(shí)，有大塊下滑，且有大塊滑移跡象，未及時(shí)采取措施，造成坍塌事故；9、 空壓機(jī)、風(fēng)鎬等設(shè)備進(jìn)場不符合要求，使用過程中造成機(jī)械傷害；10、 現(xiàn)場物品擺放凌亂，渣土及廢鋼筋未及時(shí)清理，文明施工較差；11、 現(xiàn)場應(yīng)急物資準(zhǔn)備不充分，在發(fā)生緊急情況時(shí)未能及時(shí)有效采取措施，致使事故嚴(yán)重性增加；四、洞門鑿除安全注意事項(xiàng)(1) 對各施工、操作人員進(jìn)行安全意識及操作規(guī)程教育。各施工操作人員未經(jīng)安全培訓(xùn)教育不得上崗，對特種作業(yè)必須持證上崗；(2) 現(xiàn)場施工人員一律要佩戴工作胸卡和安全帽，遵守現(xiàn)場的各項(xiàng)規(guī)章制度，非施工人員一律不準(zhǔn)擅自進(jìn)入施工現(xiàn)場。(3)為確保鑿除洞門

6、期間安全，洞門鑿除前，現(xiàn)場必須準(zhǔn)備準(zhǔn)備好砂袋、水泵、水管、方木、風(fēng)炮等應(yīng)急物質(zhì)和工具，以便發(fā)生意外時(shí)可及時(shí)封閉作業(yè)面。(4) 開工前應(yīng)檢查工具、機(jī)械的性能。防止繩索脫扣、破斷；鉆孔樁鑿除嚴(yán)格按施工方案的要求及順序進(jìn)行。(5)洞門鑿除完畢后要及時(shí)清理作業(yè)面，避免有破碎的巖塊塌落。(6)洞門鑿除期間，測量人員加強(qiáng)對洞門及地面建筑物的監(jiān)測，并及時(shí)反饋信息。(7)施工腳手架的搭設(shè)應(yīng)牢固可靠，并應(yīng)加設(shè)足夠的支撐或拉桿以保證其穩(wěn)定。高空作業(yè)應(yīng)有可靠的安全保護(hù)措施（如：按規(guī)范要求系好安全帶等等）；(8)做好用電安全教育，照明設(shè)備必須按規(guī)定配置有效的安全保證裝置。嚴(yán)格執(zhí)行施工現(xiàn)場臨時(shí)用電安全技術(shù)規(guī)范的有關(guān)規(guī)定

7、；(9)做好施工現(xiàn)場的安全防火安全工作，現(xiàn)場氣割作業(yè)要按有關(guān)規(guī)定辦理相應(yīng)的動(dòng)火手續(xù)，并設(shè)置滅火器材以防萬一。(10)在施工過程中，一旦與其他項(xiàng)目的施工（龍門吊吊裝作業(yè)等）有沖突時(shí)，立即停止洞門鑿除施工，并將施工人員撤離到安全區(qū)域；(11)施工現(xiàn)場防火、用電安全、施工機(jī)械管理及余泥外運(yùn)、散體物料運(yùn)輸、使用預(yù)拌混凝土等應(yīng)嚴(yán)格執(zhí)行國家或地方的有關(guān)規(guī)范、規(guī)程和規(guī)定，絕對禁止違章行為。(12)垂直吊運(yùn)土渣等物品時(shí)，要嚴(yán)格按照規(guī)范要求及技術(shù)交底；指揮人員應(yīng)使用統(tǒng)一指揮信號，信號要鮮明、準(zhǔn)確，吊機(jī)司機(jī)應(yīng)聽從指揮。(13)在施工過程中材料要嚴(yán)格定量使用，除合理耗損外杜絕浪費(fèi)現(xiàn)象，并采取處罰措施。(14)工地應(yīng)

8、嚴(yán)格遵守文明施工規(guī)定，并教育員工自覺遵守城市環(huán)保、環(huán)境衛(wèi)生管理?xiàng)l例，做文明員工，共同為衛(wèi)生城市作貢獻(xiàn)。(15)必須采取各種措施，限制和降低施工過程中的噪音。五、項(xiàng)目部應(yīng)急組織機(jī)構(gòu)建立以項(xiàng)目經(jīng)理何文鋒為組長，其他項(xiàng)目管理人員為組員的應(yīng)急救援小組，以便在發(fā)生事故后能及時(shí)采取措施進(jìn)行現(xiàn)場救援，防止事故進(jìn)一步擴(kuò)大。 5.1應(yīng)急救援小組組成及職責(zé)組長：何文鋒職責(zé)：負(fù)責(zé)應(yīng)急救援工作的啟動(dòng)。負(fù)責(zé)事故應(yīng)急行動(dòng)期間各單位的運(yùn)作協(xié)調(diào)，按照應(yīng)急預(yù)案合理部署應(yīng)急策略，和事故現(xiàn)場指揮者協(xié)同工作，保證事故應(yīng)急救援工作的順利完成。副組長：曾德文、李明亮職責(zé)：負(fù)責(zé)對事故現(xiàn)場的控制，協(xié)調(diào)應(yīng)急隊(duì)員的救援工作，識別危險(xiǎn)物質(zhì)及存在的

9、潛在危險(xiǎn)并對事故現(xiàn)場進(jìn)行分析，執(zhí)行有效的應(yīng)急操作，保證應(yīng)急行動(dòng)隊(duì)員的個(gè)人安全，并負(fù)責(zé)事故后的現(xiàn)場清除工作。應(yīng)急救援部門.1 技術(shù)室陳俊毅，職責(zé)：擔(dān)負(fù)應(yīng)急事故發(fā)生后的技術(shù)處理，從技術(shù)方面提供處理意見；.2 安監(jiān)室王偉，職責(zé)：擔(dān)負(fù)應(yīng)急事故發(fā)生后的安全處理工作；.3 材設(shè)室葉景雄，職責(zé)：擔(dān)負(fù)應(yīng)急事故發(fā)生后物資設(shè)備的供應(yīng)工作；.4 測量室溫振波，職責(zé)：擔(dān)負(fù)應(yīng)急事故發(fā)生后的監(jiān)測工作；.5 綜管室李毅昌，職責(zé)：擔(dān)負(fù)緊急情況下受傷人員的初步救護(hù)工作；.6 應(yīng)急搶險(xiǎn)隊(duì)，由掘進(jìn)一班、掘進(jìn)二班組成，職責(zé)：擔(dān)負(fù)施工現(xiàn)場應(yīng)急事故的處置任務(wù)。5.2、事故處理流程一旦事故發(fā)生，項(xiàng)目部要及時(shí)調(diào)動(dòng)并合理利用應(yīng)急資源，包括人力

10、資源和物資資源投入行動(dòng)；在事故現(xiàn)場，針對事故的具體情況選擇先期處置方案，從而能及時(shí)有效地使傷害和損失降低到最低程度和最小范圍。應(yīng)急行動(dòng)的優(yōu)先原則：1、員工和應(yīng)急救援人員的安全優(yōu)先；2、防止事故擴(kuò)散優(yōu)先；3、保護(hù)環(huán)境優(yōu)先。5.2.1、事故現(xiàn)場：（1）、上報(bào)事故現(xiàn)場、項(xiàng)目部負(fù)責(zé)人或安全員應(yīng)采取以下行動(dòng)：、掌握情況。不論現(xiàn)場何種局面，必須掌握的情況有：事故發(fā)生的時(shí)間與地點(diǎn)、種類和強(qiáng)度；已知的危害方向；事故現(xiàn)場傷亡情況，現(xiàn)場人員是否已安全撤離；是否還在進(jìn)行搶險(xiǎn)活動(dòng)；有無火災(zāi)與爆炸伴隨；現(xiàn)場的方向、風(fēng)速；事故危及項(xiàng)目外的可能性。、報(bào)告。在搶救、營救和撤離人員的同時(shí)，現(xiàn)場負(fù)責(zé)人必須將掌握情況迅速上報(bào)至值班

11、經(jīng)理或直接上報(bào)給項(xiàng)目經(jīng)理。、分級。值班經(jīng)理或項(xiàng)目經(jīng)理接到報(bào)告后，對報(bào)告情況按照以下情況進(jìn)行分級：一級：預(yù)警，最低應(yīng)急級別，現(xiàn)場可以控制的異常事件或容易被控制的情況。二級：現(xiàn)場應(yīng)急，屬于中間應(yīng)急級別，不會(huì)超出項(xiàng)目界限。外部財(cái)產(chǎn)一般不會(huì)受到事故影響。這種級別表明項(xiàng)目內(nèi)人員已經(jīng)不能或不能立即控制事故，需要外部援助（如消防，醫(yī)療單位的援助）。三級：全體應(yīng)急，這是最重的緊急情況，表明事故已擴(kuò)散到項(xiàng)目外。根據(jù)不同事故類型和外部人群可能受到影響，可決定進(jìn)行安全避難或疏散。同時(shí)需要醫(yī)療和其他機(jī)構(gòu)的人員支持。、值班經(jīng)理或項(xiàng)目經(jīng)理對緊急情況分級后，實(shí)施相應(yīng)的匯報(bào)工作：一級：自行組織實(shí)施控制工作，消除影響后可選擇報(bào)

12、公司以及其他單位相關(guān)領(lǐng)導(dǎo)匯報(bào)。二級：立即通報(bào)項(xiàng)目部應(yīng)急指揮中心，由指揮中心立即向公司、監(jiān)理和業(yè)主相關(guān)領(lǐng)導(dǎo)匯報(bào)，并請求啟動(dòng)公司級應(yīng)急預(yù)案。三級：立即向公司應(yīng)急領(lǐng)導(dǎo)小組、監(jiān)理和業(yè)主匯報(bào)，向當(dāng)?shù)毓蚕馈⑨t(yī)院、自來水公司、煤氣公司和工地或受影響區(qū)域所在街道的政府機(jī)構(gòu)負(fù)責(zé)人通報(bào)事故情況，請求支援，并請求啟動(dòng)縣、區(qū)、市一級或建設(shè)單位的應(yīng)急預(yù)案。值班經(jīng)理或項(xiàng)目經(jīng)理必須密切注意緊急情況的發(fā)展，當(dāng)緊急情況升級時(shí)必須按照高一級的情況實(shí)施相應(yīng)的匯報(bào)工作。（2）、現(xiàn)場救援根據(jù)不同的事故類型實(shí)施不同救援措施（具體可對照各專項(xiàng)預(yù)案開展），但必須體現(xiàn)應(yīng)急行動(dòng)的三大原則，集中力量投入先期搶險(xiǎn)，包括：搶救受傷人員和撤離危險(xiǎn)區(qū)

13、域人員到安全地帶；停止設(shè)備運(yùn)轉(zhuǎn)、斷電、滅火、隔離危險(xiǎn)區(qū)；清點(diǎn)撤出人員數(shù)量；組織力量為前來救援的隊(duì)伍創(chuàng)造條件。、應(yīng)急指揮中心：（1）、紀(jì)錄事故發(fā)生區(qū)報(bào)告的基本情況；按照預(yù)案規(guī)定，通知各應(yīng)急工作小組達(dá)到集中地點(diǎn)，并規(guī)定時(shí)限；根據(jù)危急程度或預(yù)案規(guī)定通知各工作小組做好應(yīng)急出動(dòng)準(zhǔn)備。（2）、制定應(yīng)對措施：根據(jù)事故發(fā)生區(qū)報(bào)告的情況，指示應(yīng)急處理技術(shù)組對事故影響進(jìn)行評價(jià)，會(huì)同專家咨詢判斷情況，研究行動(dòng)方案；根據(jù)行動(dòng)方案，分配各小組工作任務(wù)；根據(jù)現(xiàn)場事故發(fā)展程度，決定是否請求啟動(dòng)公司級或當(dāng)?shù)乜h、區(qū)、市一級或建設(shè)單位的應(yīng)急預(yù)案。具體行動(dòng)流程如下圖所示：情況惡化發(fā)生事故或緊急情況被人員發(fā)現(xiàn)上報(bào)應(yīng)急領(lǐng)導(dǎo)小組事故嚴(yán)重
